What you share with us
Our forms may collect your name, email address, messages, nomination details, and any photographs you choose to upload. Nominations may also include a recipient’s name, role, workplace, and workplace delivery address.
Please use workplace addresses for nominations, never home addresses. Do not include patient, student, medical, or other sensitive information. Only upload content you have permission to submit.
How we use submissions
We use nominations to review potential recipients, contact nominators, and arrange gifts. We use contact details to respond to messages and follow up about submissions. Sharing a Cali Moment is optional, and submissions are reviewed privately before anything is posted.
Permission to share your moment
We will publish your submitted message or photographs on our website or social media only if you give permission. Permission to include your first name is a separate, optional choice. Your email address and delivery details will not be published.
Before submitting a photograph, make sure everyone shown agrees to its submission and any permitted publication. A parent or guardian must give permission for a child shown in a photograph.
Where information is handled
Our Jotform forms process and store submissions and uploaded files through Jotform. Submission notifications are configured for [email protected], so information may also be held in our email account. These forms do not automatically publish a public gallery. Jotform and its security providers may process technical information to operate the forms and prevent spam. Read Jotform’s privacy policy.

How long we keep submissions
Submissions not selected for publication, including their uploaded photos and related notification emails, will be deleted within 12 months of receipt. A nomination or contact message is not permission to publish it.
For moments you authorize us to publish, we keep the selected content and its permission record while it remains published. You may ask us to remove it or withdraw permission at any time.
Questions, deletion, or withdrawing permission
Email [email protected] to ask about your information, request deletion, or withdraw permission to share a moment. Include enough detail to identify your submission; please do not send sensitive identification documents.
We will review your request and remove content from the website and social accounts we control when permission is withdrawn. We cannot remove copies other people have independently saved or shared, and service-provider backups may take additional time to expire.
Contributions
When you contribute online, Stripe handles your payment details through its secure embedded checkout. We can view contribution records such as the amount, payment status, and contact information you provide in Stripe. Monthly contributions are charged automatically until canceled. Use the management link on the Contribute page to update payment details or cancel future charges through Stripe. This website does not receive or store your full card number.
